School Physicals

All students must have a physical examination when entering grades kindergarten, two, four, seven, and ten, as well as all new entrants to the district. This is a requirement by New York State Education Law.

The results of the physical shall be valid for a period of twelve months through the last day of the month in which the physical was conducted, but shall not be more than twelve months prior to the commencement of the school year in which the examination is required.

Please note: All junior high students who participate in organized sports must have a valid physical on file with sports clearence noted. School and sports physicals are done four times a year by our school physician. If you prefer to have your own practitioner perform your child's physical exam, please bring a copy of the form attached below for your provider to complete. Please return completed form to health office as soon as possible.
